Thompson Family Practices Receive CMMI Grant

CANANDAIGUA – Thompson Health recently learned that all five of its family practices have been selected as second-year participants in the Center for Medicare & Medicaid Innovation (CMMI) grant administered by the Finger Lakes Health Systems Agency.
Funded by the Patient Protection and Affordable Care Act, CMMI grants support promising new payment and care delivery models for Medicare, Medicaid and the Children's Health Insurance Program.
Through its selection as a participant, Thompson will significantly enhance its five family practices’ ongoing transformation to Patient-Centered Medical Homes (PCMH) certified by the National Committee for Quality Assurance. The PCMH care delivery model offers a comprehensive, team-based approach to care, focused on preventative care and resulting in greater patient satisfaction and lower hospitalization rates.
The funding received through the CMMI grant will provide trained RN care managers to assist all patients within each practice, a dedicated practice improvement consultant, an additional coordinator to work with the practices on social services integration and a financial award of $440,000 to go towards the practices’ transformation to PCMH.
“We are thrilled to be able to participate in the CMMI grant, and grateful for the opportunity to enhance the nursing care coordination in our practices,” said Thompson Hospital Director of Practice Management Catherine Shannon.
Thompson has already begun recruiting for care manager positions. Its five family practices, with a total of 13 providers, are located in Canandaigua, Honeoye, Lima, Shortsville and Victor. All five are currently accepting patients. For more information, visit www.ThompsonHealth/FamilyPractice.

Thompson Receives Award for Stroke Care

CANANDAIGUA – Thompson Hospital has received the American Heart Association/American Stroke Association’s Get With The Guidelines®-Stroke Silver Plus Quality Achievement Award. The award recognizes Thompson’s commitment and success in implementing a higher standard of stroke care by ensuring that stroke patients receive treatment according to nationally accepted standards and recommendations.
To receive the Get With The Guidelines-Stroke Silver Plus Quality Achievement Award, Thompson achieved at least 12 consecutive months of 85 percent or higher adherence to all Get With The Guidelines-Stroke Quality Achievement indicators and achieved at least 75 percent or higher compliance with six of 10 Get With The Guidelines-Stroke Quality Measures during that same period of time, which are reporting initiatives to measure quality of care.
These measures include aggressive use of medications, such as antithrombotics, anticoagulation therapy, DVT prophylaxis, cholesterol reducing drugs and smoking cessation, all aimed at reducing death and disability and improving the lives of stroke patients.
In addition to the Get With The Guideline-Stroke award, Thompson has also been recognized as a recipient of the association’s Target: Stroke Honor Roll, for improving stroke care. Over the past quarter, at least 50 percent of the hospital’s eligible ischemic stroke patients have received tissue plasminogen activator, or tPA, within 60 minutes of arriving at the hospital (known as ‘door-to-needle’ time). A thrombolytic, or clot-busting agent, tPA is the only drug approved by the U.S. Food and Drug Administration for the urgent treatment of ischemic stroke. If given intravenously in the first three hours after the start of stroke symptoms, tPA has been shown to significantly reverse the effects of stroke and reduce permanent disability.
“With a stroke, time lost is brain lost, and the Get With The Guidelines-Stroke Silver Plus Quality Achievement Award demonstrates that our staff is committed to providing care that has been shown in the scientific literature to quickly and efficiently treat stroke patients with evidence-based protocols,” said Thompson Health Senior Vice President of Medical Services Dr. Carlos R. Ortiz.
“Thompson is to be commended for its commitment to implementing standards of care and protocols for treating stroke patients,” said Lee H. Schwamm, M.D., chair of the Get With The Guidelines National Steering Committee and director of the TeleStroke and Acute Stroke Services at Massachusetts General Hospital in Boston. “The full implementation of acute care and secondary prevention recommendations and guidelines is a critical step in saving the lives and improving outcomes of stroke patients.”
Get With The Guidelines-Stroke uses the “teachable moment,” the time soon after a patient has had a stroke, when they are most likely to listen to and follow their healthcare professionals’ guidance. Studies demonstrate that patients who are taught how to manage their risk factors while still in the hospital reduce their risk of a second heart attack or stroke. Through Get With The Guidelines-Stroke, customized patient education materials are made available at the point of discharge, based on patients’ individual risk profiles. The take-away materials are written in an easy-to-understand format and are available in English and Spanish. In addition, the Get With The Guidelines Patient Management Tool provides access to up-to-date cardiovascular and stroke science at the point of care.
According to the American Heart Association/American Stroke Association, stroke is one of the leading causes of death and serious, long-term disability in the United States. On average, someone suffers a stroke every 40 seconds; someone dies of a stroke every four minutes; and 795,000 people suffer a new or recurrent stroke each year.

Anthony L. Jordan Health Center Now Operating Canandaigua Clinic

A clinic that for four years has provided care to uninsured and underinsured community members will now be operated by the Anthony L. Jordan Health Center, a not-for-profit primary care center headquartered in Rochester and widely recognized as one of the first Federally Qualified Health Centers in the country.
Now called Jordan at CCIA (Canandaigua Churches In Action), the clinic originally opened in 2009 at 120 N. Main St., in the building that once housed the original Thompson Hospital. Funded in part by a grant from Excellus BlueCross Blue Shield, it was the result of collaboration between Thompson Health, CCIA and Catholic Charities of the Finger Lakes. Its staff – Thompson associates – provided short-term, non-urgent health care while patients applied for insurance and found a family physician with the assistance of a resource coordinator.
Thompson Health President/CEO Michael F. Stapleton Jr. says transferring the clinic operation to the Anthony L. Jordan Health Center – effective today, April 8 – means more comprehensive healthcare services will now be available to patients. As a Federally Qualified Health Center, Anthony Jordan is able to provide patients who have Medicaid coverage or no insurance with a sliding fee scale to access the full array of family practice services.
“Anthony Jordan has been providing affordable, high-quality primary health care for more than 40 years,” Stapleton said. “It has been Thompson’s privilege to operate the clinic for the past four years, and it is our honor to be able to entrust it to Anthony Jordan with the knowledge that this exciting transition ensures patients will continue to receive services in a manner that best meets their needs.”
Dr. Janice Harbin Joseph, interim CEO of Anthony L. Jordan Health Center, echoed Stapleton and said Jordan at CCIA will be part of the resources Ontario County families need to make total health accessible and affordable.
“Jordan never works alone,” she said. “We join hands with agencies who can offer solutions to problems facing our patients like homelessness, food and clothing needs, and inadequate transportation. Thompson and Canandaigua Churches In Action – and the entire safety net of service providers here – have extended a warm welcome to us and we look forward to being part of this community for years to come.”
Jordan at CCIA is open Monday through Friday; patients can call (585) 396-0222 for hours or to schedule an appointment.

A small group of scrapbooking enthusiasts gathered recently for the 6th Annual Croppin’ for a Cure event at Thompson Hospital, raising almost $300 for the hospital’s Colon Cancer Awareness Education Fund.
This annual, daylong scrapbooking event is organized by members of the Ambulatory Procedures Center (APC) staff, dedicated physicians and nurses who provide potentially life-saving screenings for colon cancer.
The Ambulatory Procedures Center at Thompson recently relocated to a newly renovated space adjacent to the Surgical Care Center in the new Constellation Center for Health and Healing. This space, designed with patient privacy and comfort in mind, has double the number of endoscopy suites, minimizing wait times for procedure scheduling. For more information, call the APC at 585-396-6595.

Thompson to Host Free Screenings for Oral, Head & Neck Cancer

Thompson Health’s Speech Pathology Department will host free screenings for oral, head and neck cancer on Friday, April 19, from 1 to 5 p.m. at Rehabilitation Services in the Constellation Center for Health and Healing, 350 Parrish St., Canandaigua. Screenings will be conducted by Dr. John Centonze of Finger Lakes Otolaryngology, who is donating his time to provide services at no cost to participants. This event coincides with Oral, Head and Neck Cancer Awareness Week, which begins April 14.
Those who exhibit warning signs of oral, head and neck cancer – cancers that develop in the tongue, tonsils, sinuses, larynx (voice box), thyroid, salivary glands, on the skin of the face and neck and in the lymph nodes of the neck – are encouraged to schedule an appointment for a screening. These warning signs can include red or white patches in the mouth, vocal changes or hoarseness lasting more than two weeks, a sore throat that does not subside, pain or swelling in the mouth or neck that does not subside, a lump in the neck, ear pain and difficulty speaking, swallowing and/or breathing.
Those who smoke or use chewing tobacco – behaviors related to 85 percent of oral, head and neck cancer cases – are also encouraged to schedule a screening, as are those who consume alcohol in excess. In addition, young adults displaying any of the aforementioned symptoms are urged to schedule a screening, as research has linked increased incidences of tongue and tonsil cancer in this traditionally low-risk population to the rise of human papillomavirus (HPV), a cancer-causing virus that can be transmitted through oral sex.
Registration is required; please call the Speech Pathology Department at (585) 396-6267 for additional information or to schedule an appointment. For more information, visit www.ThompsonHealth.com.
The Constellation Center for Health and Healing is located on the north side of the main Thompson Health campus and is accessible via The Thompson Way off of West Street. Once inside the Constellation Center, please follow signs for Rehabilitation Services.

Day of Dance at Eastview Mall Feb. 9

Thompson Health welcomes the community to “get their dance on” – along with over 75 other hospitals across the U.S. – to the National Day of Dance for Heart Health event. Thompson’s annual Day of Dance will take place on February 9, 2013 in Victor, New York at Eastview Mall’s Main Court from noon to 4 p.m. Day of Dance celebrates women taking action for good heart health by providing heart healthy information and by promoting exercise through dance.
Local dance and exercise experts will provide a variety of free dance lessons and demonstrations, including Ballroom, Jazzercise, Swing, Zumba, Slide Dancefit/R&B Line Dancing and more. There will be free drawings, including gift baskets from Thompson’s partners in health like Wegmans, Oliver’s Oils, Sweet Expression, Eric’s Office and many more.
Thompson’s Spirit of Women program focuses on dance as a way to take steps for a healthier life because dancing can burn as many calories as swimming, walking or riding a bike (as many as 200-400 calories per hour). Many health benefits may result from dance if it is done on a regular basis as a form of exercise. Benefits include weight maintenance or weight loss, blood pressure and cholesterol management (lowering), increased energy, decreased sadness and depression, increased coordination and muscle strength, and an overall sense of well-being. Coronary heart disease, which causes heart attack, is the single leading cause of death for American men and women. Day of Dance also educates consumers on a variety of cardiovascular conditions such as atrial fibrillation, stroke and more.
With free cholesterol testing, blood pressure testing and interactive wellness booths, experts from Thompson’s cardio-pulmonary team and many other departments will provide screenings and heart healthy information to help women and their families reduce heart disease risk factors. Medical experts from the University of Rochester Medical Center’s Finger Lakes Cardiology Associates will be available to review the results of screenings, evaluate cardiovascular risks and answer heart healthy questions. Thompson’s “Queen of Hearts” will also be at the event to educate the community about echocardiogram tests.
“This event is a great way for Thompson to reach out to the community and share heart healthy information,” says Jeanna Savage, Thompson’s Director of Spirit of Women. “It also gives Thompson an opportunity to highlight its programs and services available in the Victor area including our new Lab Draw Station, Urgent Care Center, HealthWorks occupational medicine for businesses, and our Family Practice physicians located in Victor.”
For more information about Thompson’s Day of Dance, go to www.Spirit.ThompsonHealth.com or call the Day of Dance Spirit line at 585-919-3899. To register as a Spirit member, please visit www.Spirit.ThompsonHealth.com
The 2013 National Day of Dance is presented by Thompson Health, the Spirit of Women Hospital Network, Good Housekeeping magazine and The Heart Truth ® campaign.

CANANDAIGUA – Thompson Health associates, donors and board members will gather with family and friends of the late James W. Doran Friday for a dedication in his memory.
With speakers including New York State Senator Michael F. Nozzolio and State Assemblyman Brian M. Kolb, the 10:30 a.m. dedication ceremony will involve the unveiling of James W. Doran Drive – the newly-renamed entrance to the hospital campus from Parrish Street.
Doran, who passed away in January, retired from Thompson Health in 2006 after 35 years of service as a board member and associate. He wore many “hats” over the years, including chairman of the board for F.F. Thompson Hospital, president and CEO of FFTH Properties, leader of the Thompson Foundation and chief financial officer of the health system.
“Jim’s dedication and drive were instrumental in Thompson Health’s growth; he oversaw expansion of the M.M. Ewing Continuing Care Center, the creation of Ferris Hills at West Lake, construction of the Sands Cancer Center/physicians’ office building, and expansion of the Hawks Emergency Department and Polisseni Family Diagnostic Imaging Center. As a health system, we are truly indebted to him and we welcome the opportunity to celebrate his contributions with something that will become a daily reminder of his legacy,” said Thompson Health President/CEO Michael F. Stapleton, Jr.

CANANDAIGUA – Following recent national reports of a rare fungal meningitis caused by contaminated vials of injectable steroid medication for back pain, Thompson Health officials want to assure both patients and area providers that the compound linked to the outbreak was never in use within the health system.
According to the New York State Department of Health and the Bureau of Communicable Disease Control, three sites in New York received now-recalled lots of the methylprednisolone acetate injections compounded by New England Compounding Center (NECC). These sites have all been contacted by the Centers for Disease Control and Prevention, Food and Drug Administration and/or NYS Department of Health. All other health facilities that have methylprednisolone acetate from NECC have been advised to quarantine the product.
While Thompson does provide pain management services, Thompson officials note that the product in question is a non-FDA-approved product and therefore does not meet Thompson’s safety guidelines.
“Our pharmacy is very diligent when ordering any product. We require that all medication purchased meets both pedigree and integrity standards,” says Thompson Health Director of Pharmacy Robert Locke.
“While we do not have this product within our health system and never have, we felt it
important – in light of recent reports – to assure those we care for that there is no cause for concern,” Locke adds.
Patients who received injections elsewhere are urged to contact those facilities for further information. Information about the outbreak is available at www.cdc.gov.

CANANDAIGUA – Thompson Health’s chief financial officer and senior vice president of finance is one of the Rochester Business Journal’s Forty Under 40 honorees this year.
Chosen for both professional and civic achievements, Mark Prunoske will be honored Oct. 29 during the Forty Under 40 luncheon held at the Hyatt Regency Rochester.
Prunoske, 39, is a graduate of Mansfield University and the University of Rochester’s William E. Simon Graduate School of Business Administration who first joined Thompson Health in 1997 as a financial analyst. The Canandaigua resident was later promoted to director of Finance and from 2008 to 2010 served as VP of Finance. He was named to his current position in 2010.
In addition to overseeing nearly 100 associates in 10 departments of Thompson Health, a multi-corporation community health system in Canandaigua, Prunoske implemented one of the few true cost-accounting systems within a New York healthcare system, closed on a $29.7 million bond deal to finance Thompson’s current expansion/renovation project, and secured $15 million in financing for its 132-apartment senior communities complex.
Prunoske’s community involvement includes service through the Canandaigua Salvation Army and the Rotary Club of Canandaigua.

Thompson Guild Partnership Promotes Child Safety

CANANDAIGUA – In partnership with the Ontario County Safety Council and the American Red Cross, the Thompson Guild recently launched a new child safety program called ICE (In Case of Emergency).
At child safety seat checks throughout the county, Ontario County sheriff’s deputies and other certified car seat technicians are handing out sticker sets that were provided by the Guild. These stickers give peace of mind to parents who want to ensure that their children receive the best possible care in an emergency.
The sticker sets are also being distributed to new parents through The Birthing Center at Thompson Hospital.
One sticker, attached to the car seat, contains all of the child’s pertinent health information and can be used by first responders and hospital personnel when the driver of the car has been in an accident and is unable to communicate. The second sticker in the set is a bright yellow sticker that is affixed to the outside of the car window nearest the car seat to alert emergency responders that the information is available.
For a list of upcoming child safety seat checks, visit www.co.ontario.ny.us and click on the link to the left of the screen for the Ontario County Sheriff’s Office page. To schedule an appointment for a seat inspection, call the sheriff’s Civil Office at (585) 396-4666.
ICE materials may be obtained by contacting the Ontario County Sheriff’s Office at the above number or the Canandaigua Police Department at (585) 396-5035.

At Thompson, spirituality is defined in the broadest and most universal terms possible: that which gives meaning or purpose and belonging or connection to each person. · Compassionately communicate and empathize with associates, patients, residents and their families
· Pray in a meaningful way with associates, patients, residents and their families
· Give appropriate spiritual and emotional support in medical crises
· Have a basic understanding of the nature of dementia, Alzheimer’s, congestive heart failure and cancer
· Understand the effects of stress in people’s lives in the midst of medical crises
· Give spiritual and emotional support to the dying and to families before and after death

CANANDAIGUA – A Thompson Health associate received the R. Randall Farnsworth Award at the United Way of Ontario County’s recent Volunteer Recognition Dinner, held at the Ramada Geneva Lakefront.
Kathy Perry-Ernisse was honored for going above and beyond in organizing Thompson’s 2012 United Way campaign, which raised more than $44,400, a 142-percent increase over the 2011 total raised within the health system.
The Springwater resident, who works as an office manager within the health system, was described by United Way of Ontario County Executive Director Rhoda Childs as “a consummate team player who also took personal initiative when needed to get the job done.”
“Kathy somehow conveys a bond with any effort she undertakes or assignments she is given and gives fully to it. She worked very hard this year, as she does every year, and the results definitely showed,” Childs said. She noted that Perry-Ernisse not only promoted the one-month campaign within the health system but assisted in organizing its raffle and unique silent art auction as well as United Way presentations to some 30 departments.
Also at the June 21 United Way dinner, Thompson Health was named a Cornerstone Account for raising more than $30,000 during its United Way campaign, which also resulted in the donation of a truckload of food.
“If you really want to know how to run a successful campaign, all you need to do is copy Thompson Health’s strategies,” Childs said.

CANANDAIGUA – A Thompson Health cardiac rehabilitation nurse was recently named one of only two recipients of a national award presented by Mended Hearts, Inc.
A nonprofit organization, Mended Hearts partners with more than 460 hospitals and rehabilitation clinics throughout the country, offering information and support to people with heart disease and their families.
The organization’s Rochester chapter has a Canandaigua satellite program at Thompson Hospital, and coordinator Mary Allhusen, RN-BC, was nominated by Rochester Chapter President Sharon Feldman for the Sydney & Helen Shuman Nurse Recognition Award. The award was established “to show appreciation to the nurses who make a positive difference by their skill, care and presence.”
“Mary’s patients love her and feel that she has given them confidence in their ability to recover from their cardiac problems,” said Feldman, adding that for more than 20 years, “Mary has been unrelenting in bringing hope and caring to heart patients and their families.”
A Canandaigua resident, Allhusen has worked at Thompson since 1979. She assisted with founding the hospital’s Cardiac Rehabilitation Department in 1991, supervised it for many years, and remains a staff nurse. In addition to assisting Facilitator Jim Fralick with Thompson’s monthly Mended Hearts support group, she is a key organizer of Thompson’s annual Rose Walk – which raises funding for the Cardiac Rehabilitation and Fitness Center’s scholarship fund – and helped launch Thompson’s Eastview Mall Walkers program.
Allhusen is expected to be officially presented with her award at the Sept. 13 meeting of Thompson’s Mended Hearts chapter.
The one other 2012 recipient of the Sydney & Helen Shuman Nurse Recognition Award is a nurse who works with a Mended Hearts chapter in Knoxville, Tenn.

CANANDAIGUA, NY – Thompson Health is pleased to announce the acquisition of a da Vinci ® Surgical System, a minimally invasive option to open and standard laparoscopic surgical procedures. Thompson’s surgical teams recently began using this advanced technology in the fields of urology and gynecology.
This technology offers new options for patients in our area seeking surgical treatment for prostate, kidney and bladder cancers, as well as gynecological procedures such as hysterectomies and fibroid removal. It offers such benefits as:
· Less pain and scarring
· Fewer complications
· Less blood loss
· A shorter hospital stay
· A quicker recovery period
Thompson’s urology surgical team is headed by Guan Wu, M.D., Ph.D., who has performed more than 700 da Vinci procedures at the University of Rochester Medical Center and Highland Hospital, and who has begun cases at Thompson Health. He will soon be joined at Thompson by other da Vinci-trained surgeons.
For gynecology surgical offerings, Thompson’s Chief of OB/GYN, Robert Scott, M.D., has added da Vinci procedures as the natural progression of his extensive laparoscopic experience and will be joined this summer by another da Vinci-trained surgeon.
“We are so pleased to be able to offer the da Vinci system to our community and patients,” says President/CEO Michael F. Stapleton, Jr. “We look forward to bringing even more surgical options in the near future in the areas of gynecologic oncology, colorectal cancer and otolaryngology, as well as welcoming more da Vinci-trained surgeons to Thompson.”

New Rehabilitative Equipment Available in Farmington

FARMINGTON – Thompson Health’s Farmington Rehabilitation Department is pleased to provide a new piece of rehabilitative equipment that can help jockeys, competitive equestrians and those recovering from an equine-related injury get back in the saddle.
Called the Equicizer, this non-motorized, mechanical horse simulates a real horse’s movement. Through its patented spring-designed mechanism – controlled by the user's level of effort and fitness – riders can rehabilitate, improve body posture and positioning, exercise and practice their technique in a safe, controlled environment.
“Often, our patients come to us with injuries that require them to regain strength and flexibility in order to return to the saddle. Conventional exercises used in rehabilitative therapy assist in this, but do not always recondition the areas of the body that are essential to riding,” says Physical Therapist Farley Wagner of Farmington Rehabilitation. “By rehabilitating on the Equicizer, however, patients can recondition these essential areas through simulated riding practices, increasing strength and confidence while decreasing the time it takes to return to riding.”
The Equicizer isn’t just for equestrians, though. In addition to mountain bikers and motocross enthusiasts, it can also benefit those with neurological or vestibular conditions. Through exercises performed using the Equicizer, those who experience balance problems or dizziness can improve their sense of balance and flexibility while gaining confidence.
For more information about the Equicizer or to schedule an appointment at Farmington Rehabilitation – located at 1160 Corporate Drive, Farmington – please call 585-924-4449.

Electronic System to Enhance Care at Thompson Hospital

CANANDAIGUA – F.F. Thompson Hospital will soon begin using an integrated electronic medical records system to enhance patient care by efficiently and securely documenting, managing and sharing patient data. The new system goes live within the hospital on Sunday, June 24.
“The goal of this project has always been to put the patient first, and having an electronic medical record is one way of ensuring the very best quality of care for our patients,” says President/CEO of Thompson Health Linda M. Farchione, FACHE.
Benefits of this electronic medical records system include:
· One place where medical providers can access a patient’s medical history, including allergies and medications. This gives providers a more complete picture of a patient’s medical history, with the information stored in one location.
· A secure way of sharing data from locations throughout the health system in real-time. This allows providers, from anywhere within or outside of the health system, to access to their patients’ current medical data.
For patients, this means receiving the care they need in an even more timely manner. It also means better-coordinated care, with all of their healthcare providers kept up-to-date in real-time. It also allows the patient and their family members to become more involved in their own healthcare decision making. Patients are asked to please bring their insurance cards to present at registration.
F.F. Thompson Hospital’s new system also includes laboratory, radiology and other test results, document imaging and patient history to enhance the quality of patient care and support growth.
