The overall purpose of this program is to enable participants to integrate care and communication strategies, into everyday practice to address complex patient needs. Community agencies and resources will also be present.

• An application for social work continuing education hours will be submitted to the URMC Center for Experiential Learning.
• An application for nursing continuing education hours will be submitted to ONS.

Cost: $75, includes lunch and refreshments
