Facilitated Enrollment is a program to help families and individuals apply for NYS Health Insurance Program such as Child Health Plus A and B, Family Health Plus and Medicaid.

We offer a comfortable and convenient way for people to learn about the available health insurance programs.

• Face-to-Face interview to screen for eligibility for Child Health Plus, Family Health Plus and Medicaid (by appointment)
• Assistance with ACCESS NY Applications
• Annual renewal
• Follow-up on application processing and eligibility notification
