Recovering from a serious illness or injury can be a challenge, both physically and emotionally. Post-Hospital Services at the M.M. Ewing Continuing Care Center include medically complex care and rehabilitation for those recuperating from an illness or surgery.
Our goal is to help individuals of all ages recover in the safety and security of our facility, and give them the confidence to resume their daily activities and return to the community.
Our team of caring, medical professionals work together to assess each patient's unique needs to develop a course of therapy. The Interdisciplinary Care Planning (ICP) Team, which includes medical and orthopedic specialists, develops the patient's individual care plan and monitors the patient's progress and changing requirement during treatment.
The M.M. Ewing Continuing Care Center provides a friendly environment that offers patients who want to regain their strength and independence a sense of security and comfort while they recover and rehabilitate. All patients have private rooms in an attractive setting complete with a separate entrance, lounge and private gardens. A carefully planned therapeutic center with the latest equipment and a healing garden reinforces inclusion of the whole person in the physical and spiritual recovery process.
Returning Home Together with a social worker and the other health care team members, each patient decides on the services needed after discharge. The planning may include a home visit with the team to see if special equipment or adjustments to the home living area are needed.
