The overall purpose of this program is to enable participants to integrate care and communication strategies into everyday practice to address complex needs of patients. Community agencies and resources will also be present.

  • F.F. Thompson Hospital is a an approved provider of continuing nursing education by the Pennsylvania State Nurses Association Approver Unit an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. Determination of credit is pending.
  • The University of Rochester Medical Center, Center for Experiential Learning is recognized by the New York State Education Department’s State Board for Social Work as an approved provider of continuing education for licensed social workers #SW-0005. An application for 6.0 Social Work continuing education hour(s) has been submitted.  Determination of credit is pending.
  • Attendance at the entire program is required (no partial credit can be awarded).
