My Health eRecord offers two different patient portals to let you safely and confidentially access your health information from your computer through the use of this secure portal.

FOR Thompson Health Family Practice, Rheumatology and Primary Care Patients 

Thompson Health medical practices offers My Health eRecord - Medent version, a secure online portal to access portions of your medical records, as well as communicate with your practice.

To activate your account please contact your Primary Care doctor's office to receive your unique activation code. Once you have  received the code you can create your own username, password and other login information used to verify your identity.

FOR Thompson Hospital
In-patients Admitted 4/1/14 or After 


Thompson Health offers My Health eRecord, a secure online tool for you to access portions of your personal medical record. Note that this tool is available for patients who were admitted as an inpatient or observation patient to F.F. Thompson Hospital after April 1, 2014.

Once you have completed the activation form and you have been discharged from the hospital, an email from RelayHealth will be sent to you within three business days. In this email there will be instructions on how to access your clinical information.
